A donation of $18/week ($252 for the Fall 2011 semester) is requested.

This donation is fully tax-deductible.

Any contribution over that amount will be used to augment the scholarship fund
provided by SB Family School.

To receive a receipt for your tax records when you donate:

1. Write the check payable to Art of Problem Solving Foundation.
2. Be sure to write "Santa Barbara Math Ellipse" on the memo line.
3. Print and fill out the form at www.artofproblemsolving.org/Donationform.pdf.
4. Bring your check & form to any meeting.
